Dear Manchester Teaching Primary Care Trust,

I am interested in serious event reporting in the health sector. In particular Serious Untoward Incidents (SUI's) as defined by the Department of Health. I understand the Department of Health require these events to be reported through a system called Strategic Executive Information System (STEIS).

Please confirm whether you use this or another electronic system(s) to record SUI's? Please name this system(s) if not STEIS.

Please confirm whether you record on your system(s) incidents occuring within contracted services, commissioned services (including hospital, GP, dentist, pharmcist, private etc) and whether you require SUI's to be reported to you?

If you do require contracted / commissioned service SUI's to be reported to you, please clarify and define the SUI's you require to be informed of.

Please provide, in as much detail as you are able (indicateing when inforamtion has been witheld), all the information you hold on your named electronic SUI system(s) relating to all incidents reported by or to you within the years 2010, 2011 and 2012 to date.

Yours faithfully,

Kelly Jackson

Dear Catherine Wilson,

Your response appears to only confirm that you use STEIS. What about the other questions?

Yours sincerely,

Kelly Jackson

Good questions and not very good answers. I believe all PCTs and SHAs have a statutory responsibility for Performance Management services they commission.

Would be interesting to know who is the PCTs Performance Manager? How you contact them and what is their Statutory responsibility?

Thanks for asking about SUI reporting.
