Screening information form

Jack Forks made this Freedom of Information request to NHS Blood and Transplant

This request has been closed to new correspondence from the public body. Contact us if you think it ought be re-opened.

The request was successful.

Good afternoon,

Please could you provide a copy of the current screening form used when donating blood? By "screening form" I mean the Donor Health Check that those donating blood will fill in and give answers about their lifestyle, their health, their travel history etc., before having their blood checked for haemoglobin levels.

Many thanks,

Jack
