Sandilands Investigation: IA 16767 "Trams Management of Operational Risk"

Dear Rail Accident Investigation Branch,

A 26 October 2020 City AM report by Edward Thicknesse (cf. https://www.cityam.com/exclusive-an-ongo...) revealed that TfL was conducting a Safety Audit of First Group TOL (IA 16767 "Trams Management of Operational Risk") at the time of the 9 November 2016 Sandilands Accident. The City AM report quotes a TfL representative stating "The RAIB was informed of the uncompleted audit."

1) On what date did TfL inform RAIB of IA 16767?

2) On what date TfL provide RAIB with the latest draft of IA 16767 and all the fieldwork associated with this uncompleted audit?

3) Why is IA 16767 not mentioned in any of the 4 versions of the Sandilands Investigation Report RAIB has published since 7 December 2017?
