Dear Rail Accident Investigation Branch,

Paragraph 226 of the Rail Accident Report "Overturning of a tram at Sandilands junction, Croydon 9 November 2016" (published December 2017 and October 2018) discusses a) a Report from a Croydon tram driver who "contacted CIRAS on 4 March 2013 about a concern they had about fatigue arising from TOL’s roster" and b) First Group's response.

Please tell me a) the exact date RAIB was given this CIRAS Report and b) from which counterparty (e.g.," First Group", "TfL", "member of public") this CIRAS Report was provided.
