Dear Department for Work and Pensions,

1. What are the names and street addresses for the 350 Jobcentres on CBT and IAPT, reference Budget Report 1.236 http://www.disabilityrightsuk.org/news/2...

2. Will any IAPT or CBT referral or participation be in any way mandatory and include sanctions of benefits for not taking up, continuing or finishing and CBT or IAPT provision?

3. Who is funding IAPT in Jobcentres and who will the IAPT practioners be employed and supervised by?

4. Will IAPT or CBT services (actual therapy) be provided to your customers within Jobcentres or elsewhere? If elsewhere, please share examples.

5. Please provide yours or any third party guidance or policies for the Jobcentre CBT and IAPT customer data protection, consent , confidentiality and the referral process.

6. Will CBT or IAPT service users be called patients or customers? Will they be the customers or patients of the CBT or IAPT providers?
