SAIS and Covid 19 vaccination in secondary schools

In relation to the rollout of Covid 19 vaccinations to secondary school pupils, on school premises, for the period 13th September 2021 to 13th September 2022, please provide details relating to the following:
(1) Did SAIS/NHS England maintain a full record keeping protocol, including, medical records, vaccination consent form, recorded details of matters discussed in relation to informed consent and any questions asked and answers provided for each and every child?
(2) For how long is the material referred to at (1) above retained by SAIS/NHS England?
(3) Were schools provided with any of the information/records etc referred to at (1) above and if so, at what point in the vaccination rollout/process at schools was such information sent to schools?
(4) Was the information referred to at point (1) diligently utilised and referred to by the relevant members of the vaccination team in schools in every instance of vaccination at the time of each respective vaccination procedure?
(5) Please confirm the total number of pupils in relation to which vaccination was refused.
(6) Please confirm the total number of pupils which were deemed not appropriate for vaccination due to Gillick competence not being applicable as per the Gillick competence test.
