As of the 4th December 2018, none of this information has been supplied. A review was requested in respect of non-compliance of this Freedom of Information request and since then various explanations have been provided in respect of this information not being supplied and the reply being delayed.

It is unacceptable that this information has not yet been provided. Please supply a date of when this information can be expected. Please note that this will be escalated to the ICO if this data isn't supplied this week.

Thanks for sending some information. I'd like to clarify this.

1. You have confirmed that Camden and Islington NHS Trust had no policies/procedures or guidelines in place in respect of safeguarding and conducting Mental Health Act Assessment, between 2006-2007.
2. You have stated that Camden and Islington NHS Trust did not undertake assessments under the Mental Health Act. You have mentioned that these are completed/co-ordinated by AMHPs that are employed by the local authority. You state that Dr's conducting these assessments with AMHPs are 'wearing their S12 doctor hat not their C&I employee hat'. I feel this answer is very unclear. Assessments under the Mental Health Act always include both a AMHP and a Dr. Do Dr's employed by you, conduct these assessments and if so, did they conduct any such assessments at UCLH in 2007? If one of your Dr's is wearing their 'S12 doctors hat', which organisation is paying them to wear their 'S12 doctors hat'? I honestly feel that the answer provided is far from transparent. As you will be fully aware, it is not solely AMHP that conduct these assessments, they will always include a Dr too and it's my belief that these specific Drs may well be employed by your organisation.

I have received the response below in regards to your request. I am sorry I can't help very much further.

It would be very difficult to answer these questions.

1 - I'm not sure where the old policies will be kept, Claire Barcham the Islington EDT manager used to be the AMHP training manager around 2007 and may know where the old AMHP policies were kept.
2 - In 2007 the trust staff who would have been involved with Mental Health Act assessments at UCLH in 2007 would have been the Liaison Team at UCLH and the on-call section 12 doctor.
3 - It will be very difficult to answer question 3 in particular as the data for 2007 will be in Iron Mountain if it exists at all.

Following your request for information we received on  August 8^th 2018
and the subsequence follow up emails Karen Reynolds the  Head of
Governance and Quality Assurance completed a review of your request and
her response is below. 

 

I have reviewed the initial response to the FOI and updated the response.
I have found we can provide more information than in the previous
response. I apologise that initial response was not satisfactory.  
However, I also found that the Trust cannot provide some of the
information requested for the reasons stated below:

 

1. Please supply the Policies/Procedures/Guidelines that were in place, in
respect of safeguarding and conducting Mental Health Act assessments,
between 2006-2007.

 

The Mental Health Act assessments  are then undertaken by the Approved
Mental Health Practitioner (AMHP) service which is provided by the Local
Authority (it is independent from the Mental Health Trust). I could not
find any evidence that the Trust had its own AMHP policy between 2006-07.
The Safeguarding Policies for this period are attached.

 

2. Please confirm if Camden and Islington NHS Foundation Trust undertook,
or were involved in Mental Health Act assessments undertaken at UCLH in
2007.  If so, how was your trust involved in these?

 

I confirm Camden and Islington Mental Health Trust did provide a
Psychiatric Liaison Service to UCLH and as part of this service can
request a Mental Health Act Assessment. The Mental Health Act assessments 
are then undertaken by the AMHP service which is provided by the Local
Authority (it is independent from the Mental Health Trust).

 

3. Please supply the data that shows how many people were admitted under
section 2 of the Mental Health Act for in-patient treatment at St Pancras
Hospital, who had not attempted suicide immediately prior to admission,
but then attempted suicide whilst under the care of your Trust at St
Pancras Hospital?  Please supply this data for the year of 2007.

 

The Trust had 214 admissions on Section 2 in 2007. Unfortunately we cannot
provide part of the that data requested as the Trust does not collect data
on the number of people admitted who had not attempted suicide prior to
admission. During 2007 Trust did keep data on the total number of
self-harm incidents for inpatients which includes a broad range of
self-harm behaviours:

I looked at the sequence of events since your request in August 2018 and unfortunately there were several factors that led to insufficient information in the initial response.

An temporary member of staff in the information governance team handled the FOI request and then left the Trust before the response was completed which led to a delay. The interpretation of the response by the FOI handler and service manager was that the individual records of all inpatients during that time would need reviewing to establish a response to your requests which would obviously be an unrealistic task for the Trust. In relation to your specific question about admissions and suicide attempts that was not an unreasonable assumption as we do not hold this information centrally and it would only be in patient records. However, when the senior manager then re reviewed your FOI request she gave wider consideration to each aspect of your questions and felt that we could at least partly respond to the questions with more general information we hold about self-harm using central information we hold on the incident system. The first response was then sent to you without senior sign off which is not the correct process, as a senior member of staff may have picked up potential problems with the initial response. To ensure this does not happen again staff have been reminded of the process, a more senior member of the information governance team is now handling FOI requests and the Director of Nursing is signing off all FOI requests. I again apologise and assure you that we are working on improving our performance.
