(1) As a freedom of information request, do you have any policies on roles and reporting-lines (hierarchy) of psychiatrists in BSMHFT from senior down to associate or junior level?

(2) Specifically, please can you let me know what registered and unregistered psychiatrists are posted (full and part-time) with your Lyndon (Hobs Moat) Centre: Posts / Names / Qualification level / Registration numbers where applicable?

(3) Do you have a staff tree for psychiatrists operating at this centre?

Dear FOIOffice (BIRMINGHAM AND SOLIHULL MENTAL HEALTH NHS FOUNDATION TRUST),

Thank you for this information fulfilling part of my request within due FOI timescale.

I noted that you thought it not “fair” to release personal details such as names, registration numbers or, e.g., qualification, position or contact numbers. But I would perhaps question this stance as to whether fairness applies to understanding posts of public-facing persons in public service, unless an individual has some specific reason to be exempted. It could be made clearer, and may be useful for customers (patients/carers) to understand roles and contacts within the Trust especially where this disclosure was omitted (or refused) during their work.

Perhaps I will be left to fill-in or assume some of the gaps from information already in the public domain. Alternatively, I may need to ask via an FOI asked in different way to establish other information.

Thank you for the additional information on Disciplinary Policy, and Appraisal and Medical Revalidation. I was interested to see reference to Trust’s policy commitment: “its responsibility to provide safe, high quality services, the Trust actively encourages the reporting of concerns about staff members conduct and capability”, and I trust that can apply equally to staff and public reporting of “concerns”, without prejudice.

If you don’t wish to comment on the above, I’ll close this FOI request in 7 days, as ‘information part-supplied’.

I find it intensley frustrating that information I urgently required at a certain time was not made known / was withheld / was suppressed by the Trust.

A slip-shod investigation into a negligent-care case, which resulted in my son's death 9 years ago, took nearly 8 years to be only partially admitted. The Ombudsman (PHSO) is aware that my case is unresolved with regard to discipline, follow-through and redress after finding both GP and NHS Trust negligent in varying degrees.

I have no confidence that either follow policies and procedures when serious culpability threatens professionals' livelihoods and reputations. It's a familiar pattern that ruins lives. I still await candour and transparency in why we were let down when all known intelligence in Care was ignored, then denied.
