Roadworks

Dear Cambridgeshire County Council,

I'm interested in the need for repairs to areas of roads, shortly after roadworks have supposedly left the road surface in a good state after works. (ie, a company digs the road up for some reason, does that result in the road needing repair within the next 6 months?). One such repair is : http://www.fixmystreet.com/report/404072

Has Cambs CC produced any such reports, containing an analysis of which companies/organisations leave the road surface requiring repair, in the last 5 years?

1. If so, can I please have a copy of such reports?

2. If not, can you please provide a list of all requests for permission to dig up roads, within the last 3 years, within the area covered by Cambridge City Council (or, alternatively, a convenient area which covers the City and has other areas included, if that makes the request easier to fulfil). Please include
a) the location detail that you have,
b) the name of the company requesting permission,
c) any short summary of the request (e.g. an email subject line, or similar)
d) the date of the request
e) if available, whether there were any faults with the

3. If a template form is used for requesting permission to dig the road, a copy of the current form(s) in use
