River House incident's investigation report

David Mery made this Freedom of Information request to South London and Maudsley NHS Foundation Trust

This request has been closed to new correspondence. Contact us if you think it should be reopened.

Dear South London and Maudsley NHS Foundation Trust,

An independent investigation was commissioned to investigate the incident that happened on October 1 last year at River House at Bethlem Royal Hospital.

On November 8 you responded to a previous FoI request about this investigation that 'As the investigation has not been completed, we do not have the report, and do not expect to receive and ratify the independent report until the New Year. Until the report has been received and ratified, we are unable to assess any exemptions that may apply to the contents of the final report.' (source: http://www.whatdotheyknow.com/request/ri...)

As we are now well in the New Year I assume you have received this report and have been able to assess any exemptions that may apply and I would appreciate if, in compliance with the FoI act, you would please send me a copy of this investigation report.

If some delays have occurred and this report is still not available, please inform me of when you now expect for it to become available and please send me the 'terms of reference for the review' (which were agreed last year according to http://slam.nhs.uk/media/396964/novboard...)

Yours faithfully,

David Mery

Dear Ms Kazeem,

Thank you for this clarification. It is surprising as it is highly unusual for the terms of reference of an investigation to be confidential as well.

Yours sincerely,

David Mery
