River House 2012-10-01 incident
Dear Monitor,
On the night of 2012-10-01, an serious incident occurred at the River House facility operated by the South London and Maudsley NHS Foundation Trust (SLaM).
Monitor was informed of this incident at least by a letter dated 19 November 2012 sent from the Chair Health and Adult Services Scrutiny Sub Committee, London Borough of Lambeth to the Portfolio Director, Monitor.
Can you please send me information held by Monitor about this incident.
