Dear Ayrshire and Arran NHS Board,

Please can you provide me with the following information in relation to your board:

1. A copy of the Board’s Risk Management strategy
2. Copies of other operational policies and guidance such as Risk Register policy, or similar.
3. A copy of the corporate risk register – to include as a minimum a list of titles, description of risks and risk grading.
4. Structure chart of Board which identifies where Risk Management sits, from Executive leadership through to operational responsibility.
5. An indication of Full Time Equivalents who have a responsibility for risk and a summary of the other responsibilities of these individuals. For example, any other responsibilities in relation to patient safety, incident reporting or complaints handling. Provision of the job descriptions may help in this response.
6. Copy of the Board’s Annual Risk Report for 2013/14 and details of the Committee this was presented to at year end.
7. A description of the main Risk systems and software used to support the Board in risk management.

Q1. A copy of the Board’s Risk Management strategy.

A1. Please see document 1. However this strategy is currently undergoing
review with the revised version being presented to NHS Ayrshire & Arran
Board on 2 December 2014. I have attached this revised version - paper 11
which is also available on our [1]public website.

 

Q2. Copies of other operational policies and guidance such as Risk
Register policy, or similar.

A2. Included in document 2 however new guidance is included within revised
strategy detailed in point 1 above. Document 3 refers to General Risk
Assessment Procedure.

 

Q3. A copy of the corporate risk register – to include as a minimum a list
of titles, description of risks and risk grading.

A4. Document 5 refers to Risk Management Scheme of Delegation.

A5. The Risk Management Team Whole Time Equivalent establishment is as
follows:

 

1 x Head of Risk Management and Unpacs (split role between Risk Management
and Patient Safety). Document 6 refers to job description .

1 x Risk Management Lead Facilitator. Document 7 refers to job description

1x Datix Trainer. Document 8 refers to job description

1 x Risk Management Systems Software Information Facilitator. Document 9
refers to job description

2 x Risk Management Support and Development Officer. Document 10 refers to
job description

 

Q6.  Copy of the Board’s Annual Risk Report for 2013/14 and details of the
Committee this was presented to at year end.

A6.  NHS Ayrshire & Arran commissioned a significant systematic review of
its risk management system and processes in September 2013. This work is
still undergoing and the results being evaluated. Once a report is
available we would be happy to share it.

 

Q7. A description of the main Risk systems and software used to support
the Board in risk management.

A7. We use Datix. Datix is web-based patient safety software for
healthcare risk management applications. For more information, please see
the [2]Datix  website.
