Under freedom of information I have questions over Risk Management

1 What is GNHSB Risk Management Strategy.
2 What are the other operational policies and guidance such as Risk Register policy or adverse event and incident management policy.
3 What are list of titles of risk, description of risk and risk gradings.
4 Is there a structure chart of Board which identifies where Risk Management sits from executive leadership through to operational policy
5 Where does patient safety, incident reporting or complaints handling sit in these procedures and who would have operational responsibility and job descriptions
6 Can I have sight of Board's Annual Risk Report for 21014/2015 and details of the committee this was presented to.
7 Provide a description of the main Risk systems and software used to support the Board in risk

Thank you for your request received on 27 May 2015 for the provision of
information on risk management.   We are treating your request under our
procedures for responding to requests for information under the Freedom of
Information (Scotland) Act 2002 (FOISA).

I can confirm that NHS Greater Glasgow and Clyde (NHSGGC) holds the
information as described by you.

Below are the seven questions you asked and NHSGGC’s response in italics
and bold.

 

1.      What is GNHSB Risk Management Strategy.

          [A copy of NHSGGC’s Risk Management Strategy is enclosed with
this response which descrives the remit].

 

2.      What are the other operational policies and guidance such as Risk
Register policy or adverse event and incident management policy.

          [Other policies enclosed with this response are as follows:

·            Significant Clinical Incident Policy].

 

3.      What are list of titles of risk, description of risk and risk

response which contains the information requested].

 

4.      Is there a structure chart of Board which identifies where Risk
Management sits from executive leadership through to operational policy

          [This information is contained within the Risk Management
Strategy].

 

5        Where does patient safety, incident reporting or complaints
handling sit in these procedures and who would have operational
responsibility and job descriptions.

          [All staff within NHSGGC have a responsibility, either directly
or indirectly, for management of risk and patient safety and for making
safety at work a priority to protect themselves, their colleagues,
patients, visitors and the interests of NHSGGC; it is therefore an
organisation-wide responsibility.  The Risk Management Strategy describes
this in more detail (see Section 4)]. 

 

          I have included job description for posts for the undernoted
senior managers who have a special responsibility for risk (ie those
involved in patient safety, incident reporting or complaints handling)
which may be useful:-

 

·            Complaints Manager

·            Head of Clinical Governance

·            Clinical Risk Manager

·            Head of Health and Safety

·            Health and Safety Manager].

               

6        Can I have sight of Board's Annual Risk Report for 21014/2015 and
details of the committee this was presented to.

presented to the NHS Board, is included with this response].

 

7        Provide a description of the main Risk systems and software used
to support the Board in risk.

this response].

 

I hope this information is helpful.  If you are not satisfied with our
response to your request, you have a right to request a review of this
decision within 40 working days of receiving this response.  The procedure
for consideration of a review is detailed in the attached note.  The note
also describes your right to pursue the matter with the Scottish
Information Commissioner if, following a request for review, you remain
dissatisfied with the decision of NHS Greater Glasgow and Clyde.  If
following appeal to the Scottish Information Commissioner you still remain
dissatisfied with the outcome, you have a right of appeal to the Court of
Session on a point of law against the decision of the Scottish Information
Commissioner.
