Dear Health and Safety Executive,

Please can you provide the results of any risk assessments that the HSE has conducted regarding well integrity at all wells in the oil and gas development sites at Horndean, Singleton and Markwells Wood.

Please can you provide details of site monitoring and inspection of the Markwells Wood 1 and Horndean oil site.

Do you have any data or details of the well stimulation methods used at Markwells Wood or Horndean with regard to well operations (pressures used , fluid volumes during acidisation process)

With regard to Singleton, please can you provide data and analysis of the well integrity failure that occurred. If it is not classified as well integrity failure can you please provide any data or details of the incident, in particular:
1. When the well was drilled
2. When the problem was discovered
3. Data on how this incident was reported with date reported
4. Data on HSE inspection--dates site was visited
5. Correspondence between HSE and EA with regard to incident
6. How situation was remediated
