Dear Ministry of Defence,

Can you please send me a copy of the RG8 medical declaration form given to army enlistees to complete before they join?

Can you also please tell me, in addition to the information provided on the RG8 form and in the pre-enlistment medical, whether it is routine to administer diagnostic screening questionnaires for mental health problems before a person enlists, and if so, which such questionnaires are included?
