Response to this request is long overdue. By law, under all circumstances, NHS Stafford and Surrounds Clinical Commissioning Group should have responded by now (details). The person who made the request can complain by requesting an internal review.

Dear NHS Stafford and Surrounds Clinical Commissioning Group,

Please could you explain why there have been 9 versions of the original report sent to you by NWT trust in November 2018. Given that there has been minimal changes to the report.

Could you explain in detail why a report you received in November 2018 has only just been released to the public.

Could you explain why the CQC were not made aware of the safety issues within that report immediately, and why it was only sent to them last week when it was received in November.

Could you explain why a letter dated July 2018 from the independent reviewers that suggested more immediate action was taken to address the concerns found was not shared with the CQC.

Would a legal challenge around the review report prevent the CCG from sharing significant safety concerns with the CQC?

The safety issues within the report have not changed following that legal challenge.
