Under the Freedom of Information Act 2000, I kindly request the following information:
1. Inadequately Resourced Multi-Disciplinary Team (MDT) Meetings:
Records or reports highlighting concerns about inadequately resourced MDT meetings within the Trust.
Steps taken by the Trust to address such resource issues and ensure effective MDT operations.
Any documented instances where the lack of adequate resourcing in MDT meetings led to adverse outcomes or delays in patient care.
2. Failures to Disclose Trust Investigation Policies:
A copy of the Trust’s current policy or policies governing internal investigations (including clinical incidents, complaints, or governance reviews).
Any records or correspondence documenting instances where the Trust failed to disclose these policies upon request.
Procedures in place to ensure transparency and accessibility of such policies to patients, families, and other stakeholders.
If the request exceeds the cost limit, I would appreciate advice on refining the scope.
