Provide details of responsibilities of Shirley Green and/or Personnel Dimensions with regard to each contract

Provide details of how access to the final reports will be facilitated

Provide names of NHS staff who requested the use of Shirley Green and/or Personnel Dimensions to perform work

Provide names of all NHS employees and Non Executive Directors and /or Board members who were associated with approval of the use of Shirley Green and / or Personnel Dimensions to perform these investigations

Provide names of all NHS employees and non Executive Directors and /or Board members who were associated with approval of payments to Shirley Green and / or Personnel Dimensions

I request a waiver of all fees for this request. Disclosure of the requested information to me is in the public interest because it is likely to contribute significantly to public understanding of the operations or activities of the government and is not primarily in my commercial interest. Public funds were used to pay for investigations into whistle blowing concerns, bullying and harassment, and discrimination. These claims were substantiated during the investigation by Shirley Green and/or Personnel Dimensions as detailed in the final report.
