Request of numbers of children referred to Social Services via the childrens Outpatient Clinic

Dear Oxford University Hospitals NHS Trust,
I have several questions about the children's hospital to ask.

1) How many referrals have been made since 2012 to social services (for abuse/neglect allegations) via the childrens hospital, and more specifically the paediatrics outpatients clinic?.

2)
How many referrals were made by each paediatric consultant?

3)
How many of these referrals resulted in the children being placed on a child protection plan?

4)
How many of these referrals had cases of Ehlers Danlos Syndrome or Lyme Disease that the children had or were later diagnosed as having?.

5)
How many of these cases went to the ombudsman for complaint?

6)
Does the hospital put into practise any policies that allow parents a second opinion if they are not happy with the service provided, this without any referral to social services?.

Dear Ms. Potter,

 

Thank you for your email sent on the 10^th February 2017. The Trust  does
not have an electronic system that tracks referrals to Social Care. The
information is not readily available and will require an audit to enable
data extraction, which will exceed the cost limit under s12 FOIA of £450
over 18 hours @ £25 per hour.
