I refer to the news release on page 10 of the Weston, Worle and Somerset Mercury on June 27 from the CCG. The press release refers to a review of the processes by which nurse led health needs assessments are used to support decision making.

Could I please have the following information:

1. terms of reference and expected outcomes of the review
2. date review commenced
3. name of senior staff member leading review
4. minutes of any meetings held by the review group/ lead staff member
5 completion date for the review
