A report was completed by Staffordshire Healthwatch at your request and
presented to SSSFT (now MPFT) in December 2017. The report is entitled
'Evidence relating to issues and concerns regarding autism, ADHD and CAMHs
assessments and support'. In May 2018 that report was shared by SSSFT to
the CCG. I request to see a copy of the report.

After liaising with the service unfortunately, we do not have a copy of
the final report. The only copy provided to us was the copy I have sent
you previously.
