I am seeking to receive information regarding the Tier 3 Weight Management service that 
is available to your patients.
 
  
Could I request answers to the below questions: 
  
What is the name of the Tier 3 Weight Management Service available to your patients?  
 
4 healthy weight is tier 3 weight management service which is commissioned by Kent County 
Council (KCC). 
  
Do you commission the Tier 3 Weight Management Service?  
If not, who does?

What number of patients have been referred to this service in the last 12 months of 
available data?  
 
This would be data that would be held by KCC who can be contacted using the following details:
