Thank you for your response to my request for information made under the provisions of the Freedom of Information Act, however, your reply seems to advise me that in order for my request to be dealt with I need to complete a form and post it to you.
I strongly suggest that you seek proper legal advice on this position that you are taking, you may find that contacting the information commissioners office for assistance would also help you to deal with my request in an appropriate manner.
To be clear, I will not be completing any additional paperwork, my request was very clear and complies with the Freedom of Information Act.

I am writing to request an internal review of Liverpool Community College's handling of my FOI request 'Request for information stats'.
On 25 August 2012 I submitted to your organisation a request for information, in accordance with the provisions of Freedom of Information legislation.
On 28 August 2012 you replied, stating that I must complete a form, which I then had to return to the college. The language used in your email clearly gave the impression that in order for my request for information to be valid, I had to complete a hard copy form.
On 28 August 2012 I responded by advising that I had submitted a valid request for information and that I hoped you would comply with the requirement s of Freedom of Information legislation.
Sadly, you have not provided me with the information, or issued with me with a refusal notice. I therefore request that an internal review of the handling of my request for information is undertaken as soon as possible.
Once again, I would encourage you to seek proper legal advice if you are unclear on how to deal with requests for information in relation to Freedom of Information legislation, you may find that contacting the Information Commissioners office for assistance would also help you to deal with my request in an appropriate manner.
I look forward to hearing from you in due course.

(b) The number of Freedom of Information, requests for information
