FOIRequests, Health and Safety Executive

Hi Dennis

In order for this to be a valid FOI request you need to supply a full name, not just a first name. Without this you would have no recourse to the Information Commissioner if you were dissatisfied with HSE's response. Please can you confirm this and we can then process your request.

Regards

Helen

Dear Health and Safety Executive,

Could you let me have details for any reports by Associated British Ports, Dow Corning, Bomar Quest and just very short confirmations of all others your refer to where I would only need to have the name of the company reporting. the date and a very short description of what was reported. That might be sufficient but at the least it would allow me to be more specific if more is then needed.
I look forward to hearing from you again.
Yours faithfully,

dennis clarke

Dear Health and Safety Executive,

thank you for the disclosure sent to me.

There may be material missing as the request was wider than incidents where injury was caused. I included in the request material for potential incidents which I suppose could have been better worded so as to limit the ambit of that part of the request.

As an example of what I was looking for under that heading it is believed locally that in 2017 a ship carrying chemicals was 'stuck' in the lock gates due to a failure to adequately dredge. Notifications would have been sent out due to the potential for serious impact. The ship was moved before it broke and the serious incident was avoided, just.

I assume such matters will be run by the HSE. Could you check to see if there is a note of this and similar incidents. I hope to receive additional disclosure in due course.

Yours faithfully,

dennis clarke
