Report to Area Coroner for Essex following Inquest into the death of Chloe Hunt
Dear East Suffolk and North Essex NHS Foundation Trust,
I write to you under the Freedom of Information Act 2000 to request a copy of a report which was requested of the trust by Essex area coroner Sonia Hayes.
There were eight areas of concern listed by the coroner and a response to that Prevention of Future Deaths report was due to be submitted by ESNEFT by August 12, 2024.

Dear Sir / Madam,
To clarify, what I am requesting under the Freedom of Information Act 2000 is a copy of the report which was sent by the Trust to the coroner. The corner set a deadline of August 12, 2024 for the submission of this report.
This is what was requested by the coroner: "Your response must contain details of action taken or proposed to be taken, setting out the timetable for action. Otherwise, you must explain why no action is proposed."
This is the report I am requesting, that is, the report prepared by the Trust and submitted to the coroner.
