Response to this request is long overdue. By law, under all circumstances, Guy's and St Thomas' NHS Foundation Trust should have responded by now (details). You can complain by requesting an internal review.

Dear Guy's and St Thomas' NHS Foundation Trust,

Please provide number of monthly referrals to your Heart Failure Diagnostic Clinic since January 2018.

Yours faithfully,

Joel Smalley

Dear Sir/Madam,

Thank you for your e-mail. Given the current situation with the second surge of the coronavirus (Covid-19) outbreak we are taking steps to protect our patients, staff and any visitors. This means we have very limited resources to respond to requests for information and ask that you consider the following before making a request.

If you are making a Freedom of Information request please consider if this is vital at this point in time and if you can cancel or delay asking for information until we have returned to normal service. If your request is essential please consider requesting the minimum information possible. We are unlikely to be able to respond in a timely manner as the majority of requests are answered by the clinical and operational staff within the various departments in the Trust. These are often the same staff who are busy dealing with patient care, directly treating and managing Covid-19 patients or are supporting these staff and the service in the care of patients.

Please let us know if you have any further queries or information related to this request. Our reference number for the request is FOI 104218; please quote this on all correspondence.

Thank you for your consideration. If you have any further queries, please contact the Information Governance department at [email address].

Yours faithfully,

Freedom of Information Team

Dear Joel Smalley,

Would you please clarify the following in relation to your recent Freedom of Information Request 104218?

"What they mean by ‘heart failure diagnostic clinics’ and whether they are referring to ERS (GP) referrals? It would be helpful to also point out that we only have data back to Nov 2020. Please also clarify:
- Would they like all referrals received (or only those which are accepted)?
- Would they like us to make an assumption about the breakdown between Heart Failure and Inherited Cardiac Conditions or provide the total for both (as we only have the total number – and so any breakdown has to be based on an assumed %)
Please provide as much detail as possible so that we can fully understand your request. If we do not receive any clarification within three months, we will assume that you no longer require the information requested."

Yours sincerely,

Freedom of Information Team

Dear :FOI,

If you only have data back to Nov 2020, it is not useful. Please close the request.

Thanks.

Yours sincerely,

Joel Smalley
