A full list of all premises who have the sale of alcohol by retail on their premises. The list needs to contain the following:

Premises Name
Premises Licence Holders Name
Full Postal Address
Telephone number for Premise
Designated Premises Supervisors Name

Requested Format:

Please provide this information in an Excel spread sheet (.xls) format. If you cannot supply the information in this format please can you explain why.
