Please could you provide me with the following information:

1. How do you record deaths that occur while patients are on the waiting list for a first appointment at the West of England Specialist Gender Identity Clinic (based at the Laurels).

2. How do you investigate these deaths to learn from them in the future.

3a. Do the investigation policies into deaths at The Laurels differ between (A) Patients currently awaiting a first appointment and (B) patients who have had their first appointments.

3b. Policies, meeting minutes, or other such data explaining how this decision was made, as is reasonably accessible.

4. Numbers from 2017-2024 of those who have died whilst awaiting a first appointment at The Laurels.
