Re-drafting and destruction of Commissioned Reports

Dear South Western Ambulance Service NHS Foundation Trust,

I understand it is policy that "The process for the management of commissioned investigation reports is that once a final version has been agreed, any previous drafts are destroyed to ensure only the approved version is retained and actioned" Kevin Pointer, SWASFT Information Governance Manager 2018.

Professor Duncan Lewis was recently commissioned to produce a 'Cultural Review' for SWASFT (October 2018). I would like to know if the report subsequently released to the public is Prof. Lewis's original report or a re-drafting of his original report.

If it has been re-drafted, how many times and on what dates were earlier versions deleted?

Thank you for your request for information regarding the Cultural Revuew
which we received on 11^th November 2018. We have dealt with this as a
Freedom of Information request and I attach a leaflet containing further
information about both your rights and our obligations under this
legislation.

 

We are able to provide the following information:

I am writing to request an internal review of South Western Ambulance Service NHS Foundation Trust's handling of my FOI request 'Re-drafting and destruction of Commissioned Reports'.

My request of 11 November 2018, has clearly not been taken seriously as your response is unintelligible.

For your convenience, and clarity, below is my original request, clearly setting out the information I requested.

I am very sorry, there seems to have been a formatting issue  - we replied
to you in table format however this does not appear to have been
compatible with the ‘What do they know’ hosting site.  

 

As such the response we provided has been replicated for you as below;

 

The final report (‘Workplace Culture at Southwestern Ambulance NHS
Foundation Trust’)was provided directly by Professor Duncan Lewis to the
Trust. No editing was undertaken by the Trust. The copy of the report
available in the public domain is the copy the Trust was provided with in
the first instance.

 

Any requests for previous drafts of the report completed by Professor
Lewis should be directed to him as the author in the first instance.
