With reference to the recently discontinued program of targeted reassessment of
individuals with complex needs care packages to save £1 million per annum,
as conducted since 2014 ,as part of the Western Bay Regional Partnership
Board initiative.--- ( Along with NeathPortTalbot Council, Swansea
Council, and ABMU Health Board. --- saving in excess of £5 million to
date ---and ongoing .)

( Program confirmed discontinued with the transfer of Bridgend to the Cwm Taf region.)

1. Please identify the original meeting where this program was first
discussed and implemented and provide a copy of the recorded minutes.

2. What criteria was used to identify and select the individuals with
complex needs for re-assessment.? -- and who was responsible for this
identification.?

3. How many of these existing care packages were individuals who qualified
for NHS Continuing Care support.?

4. Who carried out the reassessment of the complex needs care individual.

5. What was the final total of monies saved by the Bridgend re-assessment program.

6. What was the total number of re- assessed persons carried out under this program, and how many more individuals remain to be assessed under the program , but now will not be re-assessed as a result of the transfer to Cwm Taf region.

We refer to your email dated 4^th June 2019 which contained a request for
information as set out in italics below.  Following consideration of your
request, in accordance with the Freedom of Information Act 2000, I respond
to each point as follows:

 

With reference to the recently discontinued program of targeted
reassessment of individuals with complex needs care packages to save £1
million per annum, as conducted since 2014 ,as part of the Western Bay

1.    Please identify the original meeting where this program was first
discussed and implemented and provide a copy of the recorded minutes.

The original business case was developed by the Western Bay Contracting
and Procurement Lead with the project team and was presented to the
Leadership Group in July 2014 for approval.

 

In December 2015 a new business case was presented and approved by the
Leadership Group to continue the project for a further year (2016/17)

 

The main aims of the project as agreed by the leadership group were:

·           To effect a sustainable and efficient ‘practice to
commissioning’ methodology across Western Bay which commissions high
quality health and social care services which are proportionate to need
and are cost effective.

·           To enable sharing and coordination of information,
intelligence and planning together in service area of common interest.

·           To help partners shift front line practice towards the
requirement of the Social Services and Well Being Act 2014

 

2. What criteria was used to identify and select the individuals with
complex needs for re-assessment.? -- and who was responsible for this
identification.?

 

Each group leader/team leader/principal officer were requested to identify
the individuals that required assessment. High cost packages of care were
given as criteria for consideration.

 

3. How many of these existing care packages were individuals who qualified
for NHS Continuing Care support.?

 

BCBC only identified packages of care where individuals jointly fund or
fully fund themselves. ABMU had responsibility for identifying NHS
Continuing Care packages of care.

 

4. Who carried out the reassessment of the complex needs care individual.

 

The assessments were carried out by qualified social workers.

 

5. What was the final total of monies saved by the Bridgend re-assessment
program.

 

BCBC saved £506,199 in total from the re-assessments of high cost cases
only since 2014.

 

6. What was the total number of re- assessed persons carried out under
this program, and how many more individuals remain to be assessed under
the program , but now will not be re-assessed as a result of the transfer
to Cwm Taf region.

 

BCBC have continued with the approach of this project, to undertake
reassessments with everyone who BCBC commission care packages for in
supported living. Many elements of this approach are continuing with
regard to the annual review process for adults.
