Racism complaints

Dear British Broadcasting Corporation,

It is my understanding that in the wake of the George Floyd death in the USA in May 2020 there was an initiative set up to allow staff at BBC Sport to report to managers instances they had witnessed or experienced at work of racist behaviour. My questions relate to this initiative.
1. Please provide me with a copy of any material that was sent to staff to alert them of this initiative which explained how they should report such instances?
2. How many such reports were compiled by the initiative? How many of these related to racist behaviour/comments/actions by BBC staff to other BBC employees.
3. For each of the first 25 such complaints that were logged please provide me with an explanatory summary giving the precise nature of the racist behaviour and an explanation of the setting as well as the people involved. Please note I accept that some details will need to be anonymised to comply with S.40 of the Act but that does not preclude you from giving me a description of the allegation.
4. In relation to all the allegations/claims reported where the perpetrator was said to have been an employee of the BBC could you provide a table showing what if any disciplinary measures were taken against them.
