Questions to University Hospitals Bristol NHS Foundation Trust - prescription of Avandia/Rosiglitazone
I am writing to you to request information under Freedom of Information 2000 Act.
MY REASON FOR ASKING ABOUT ROSIGLITAZONE/AVANDIA
Please note that I am conducting research into the use of Avandia/Rosiglitazone as I am very concerned about drug use in the UK and "off label" prescribing which is legal but may not be advised by NICE. I am seeking a national review of how drugs are licensed and thus protect both the prescribing doctors and pharmacists and hospitals and PCTs at the same time as optimising patient care and healthy outcome.
Therefore, I am making my request for information.
1] When did University Hospitals Bristol NHS Foundation Trust medical staff last prescribe Avandia/Rosiglitazone?
2[ When did University Hospitals Bristol NHS Foundation Trust pharmacist last dispense Avandia/Rosiglitazone?
3] Has University Hospitals Bristol NHS Foundation Trust's medical staff - including cardiologists - ever prescribed Avandia/Rosiglitazone was to a patient with a history of heart failure which was known to be "off label"? I ask this because NICE guidelines in 2004 state that Avandia/Rosiglitazone was not to be given to such patients.
3] Have you had any complaints that University Hospitals Bristol NHS Foundation Trust has prescribed Avandia/Rosiglitazone "off label"?
4] Have you had any legal action initiated against University Hospitals Bristol NHS Foundation Trust for prescribing Avandia/Rosiglitazone "off label"?
5] Have any of your patients died in consequence of side-effects of having Rosiglitazone/Avandia? And, if so, how many - if any - were known heart patients at the time of taking Rosiglitazone/Avandia?
