Please can you provide the following details in regards to Healthy Lifestyle Services -
• Stop Smoking services
• NHS Health Checks
• Weight Management
• Physical Activity
• Health Trainers (or similar model)

Are any/all of these services outsourced or delivered in-house?

Are they integrated into one service or are they outsourced separately?

If they are outsourced, please provide the following information:

This covers general recorded information that is held by the Council.
Examples: budget information, staff structures, details of contracts etc

From the next working day that the request is received we have 20 working
days to respond. This does not take in to account any clarification we may
require or if we need to consider the Public Interest Test which allows us
up to a maximum of 20 additional working days.

Environmental Information Regulation
This covers all environmental information held by the Council.
Examples: Planning Applications, details of proposed road works, flood
information, pollution information etc.

From the next working day that the request is received we have 20 working
days to respond.

Section 7 of the Data Protection Act (Subject Access Request)
This section of the Data Protection legislation only relates to requests
for information about the requester or a client if you are acting on their
behalf.
Examples: Personnel file, Housing file, Council Tax/Housing Benefit
information etc.

We have 40 days from the date of receipt to respond.

With regard to your Freedom of Information request received on 9 November
2017, please find our response below.

 

 

Please can you provide the following details in regards to Healthy
Lifestyle Services -

 

- Stop Smoking services – Outsourced

- NHS Health Checks – Outsourced

- Weight Management – Outsourced

- Physical Activity – There are some activities available through Hull
Culture and Leisure (HCAL) with funding provided out of the Public Health
Grant

- Health Trainers (or similar model) – This service is not provided

Stop Smoking services

£670,000 per annum (included NRT expenditure) provider is City Health Care
Partnership – 3 years + 1 – contract ends October 2018.

 

NHS Health Checks

Primary Care – Investment across GP Practices and Pharmacies in individual
contracts is based on a per check cost.  In 2017/18 with proposed savings,
the proposed Public Health budget value for Primary Care NHS Health Check
activity is £80,000.

Community Outreach Service - £37,500.

Workplace Health and Wellbeing Service - £50,000.

 

Weight Management

£400,000 per annum (Family WM Service and Adult WM Service), the provider
is ABL – 3 years + 1 – contract ends October 2018.

 

Physical Activity

Via the Public Health spend HCAL provide Tonic Cards to support behaviour
change.  The current value for 2017/18 is £11,000 and a one off project. 
HCAL also have an Older Peoples project for £35,000 which is another one
off project and ends March 2018.

 

In relation to renewal of contracts, Hull City Council and Hull Clinical
Commission Group have developed an integrated commissioning model which
will include pooled resources and joint commissioning via the Place Based
Plan.
