Dear Driver and Vehicle Licensing Agency,

Hello,
I applied for provisional licence form month ago online but still waiting for it, I already paid for it and I received email for that also, they give 2 estimated weeks but did not receive my form. Please send me my form or my money back. I don't have any other option to contact with Dvla.
