Dear Kent Community Health NHS Foundation Trust, I would be most grateful if you would provide me the details as below in respect of the contract:
- stare t date and duration of the current contract
- value of the current contract
- which company was awarded this contract
- is there an extension clause in the contract and if so the duration of the extension
- has a decision been made to extend the contract
- who is the procurement lead on the contract
- who is the clinical lead on the contract
- how many patients are receiving a home delivery service
- is the delivery service to the patient separate to the supply of product and if so who provides this service
- what is the value of the delivery element if funded separately
- was there a separate tender for the delivery service element
- who is the senior officer outside procurement responsible for this contract and what are their contact details
