Dear Requester

We write further to your request for information under the Freedom of
Information Act received 22nd July 2021.  We have set out your request,
together with our response below.

1) Can a person decline their private and confidential information/data
being shared with a Health Visiting Service?

Yes a person can decline, however this is also shared with the GP and
Health Visitors are assigned to a GP practice and have access to their IT
systems. The hospital have no control over the access rights to patient's
records within the GP practice.

2) If consent is not provided will a Health Visitor still be notified of a
discharge from hospital nevertheless?

Discharge information must be shared with the Health Visitor and GP for
the baby - this is shared care as the midwife will discharge mother and
baby at 2-4 weeks after birth. The Health visitor will need to ensure that
follow up for the baby vaccination programme and 6-8 week health check are
completed. This is within their duty of care.

3) How to opt out of data being shared with a third party Health Visiting
Service when their services are not required or consented to?

Health Visiting care is required as mentioned above. This is outside of
the scope of a midwife and also GPs no longer offer routine appointments
for postnatal women.

As we have provided the information that we do hold your request is now
closed.  We trust that this is satisfactory but if you are dissatisfied
with the way that it has been handled you have the right to ask for an
internal review.  Internal review requests should be submitted within two
months of the date of receipt of the response to your original letter and
should be addressed to: Geoff Stokes, Director of Corporate Affairs, UHCW,

the right to apply directly to the Information Commissioner for a
decision.  The Information Commissioner can be contacted at: Information
Commissioner’s Office, Wycliffe House, Water Lane, Wilmslow, Cheshire, SK9

Yours sincerely
