As requested the information has been provided in spreadsheet format. There are three worksheets - one for requests from 1st Jan 2005 to 31st December 2008 and one each for 2009 and 2010 (up to 31st July).

Please note that the records held in the older database (2005 - 2008) were not recorded in the same way as has been the case since 2009, in that the whole request was recorded as stated, and there was no field used for a summary. I have therefore redacted it where personal information was given.
