Prevention of future deaths recommendations on improved care for vulnerable individuals with complex needs

Please indicate how many inquests in each year from 2010 until the present day produced prevention of future deaths recommendations that were sent to you recommending or requesting that consideration be given to the provision of a) better holistic or joined up care b) additional resourcing and/or c) improved inter-agency communication in the care of vulnerable individuals complex needs, whether or not they were under the care of NHS services at the time of their deaths.
Please also indicate the name of the coroner in question and the location in which the inquest took place.
