Dear NHS Wiltshire Clinical Commissioning Group,

I had a formal pre arranged meeting at NHS Wiltshire CCG offices on 04 August 2015,
where a SIGNIFICANT number of issues were discussed , audio recorded and transcribed by yourselves , that would require YOUR IMMEDIATE PROMPT ATTENTION .

I have very generously allowed you THREE MONTHS to deal with these matters , or at least apologise meaningfully and properly IN PERSON - FACE TO FACE and in detail in writing - but nothing !

Ive requested ongoing support at the meeting due to a mental health injury and being classed as vulnerable , but this has been refused ?

I shall not list the VERY SERIOUS MATTERS raised by me at that meeting, but as THREE MONTHS has now gone by since that meeting and I have not received ANY update from your CCG, I wish to gain a full understanding of all / ANY ? processes you are following in dealing with the SIGNIFICANT NUMBER of serious matters i raised at that meeting.

There will be your own policies and procedures, information from the commissioning contract you have with the Avon and Wiltshire Mental Health Partnership, within your safeguarding policies and procedures, within your DPA and FOI Policies and Legislation, duty of candour and much much more - you are the supposedly the experts on these matters - not me so can I shall rely on you to supply me with ALL the information I am seeking ?

I wish to see every piece of information you need to abide to in regard to all the matters I discussed with Dr Peter Jenkins your chair, and your Chief Officer Deborah Fielding , with Diana Hargreaves taking what appeared to be selectively partial notes , that I understand were subsequently destroyed despite my being offered a copy at the meeting ?

To provide a proper response to this FOIA request, you will need to carefully listen and take notes throughout to the entire 95 minutes audio recording of the meeting before you can provide me with all the information I am seeking and entitled to under FOIA .

Regretably , the transcipt has numerous gaps and misleading mis spelling which clearly intoduces elements of failure to handle the information I gave you as you should, and not even to identify yourselves the failings within the transcription you prepared or to contact me to put it right . Doesn't really install any confidence in your CCG does it ?

I wish to receive all the documents you hold regarding how you should be dealing with the matters I have raised in careful detail with you during the meeting - which I first asked your previous chair to arrange 2 years ago here on WDTK - but he ignored me !

Will someone senior, preferably Dr Peter Jenkins please TOTALLY update me BY RECORDED DELIVERY POST in line with ALL NHS requirements, I trust WITHOUT FAIL you will ensure I receive a comprehensive and detailed update on ALL the actions you have taken over the past THREE MONTHS since you were formally alerted to these very serious matters ? Is SEVEN DAYS an acceptable timescale in light of your three months delay ?

However I wish you to provide the FOIA information relevant to my request here on the whatdotheyknow website within the Legislatory timescale please - Ive already received far too many ' so called apologies ' from your CCG for repeatedly failing to comply with my Rights under FOIA - and DPA ,many of which appear here on WDTK - but also the insensitive bullying ' parting shot ' from your former Chair Dr Stephen Rowlands.

I had hoped things would improve, progress and be resolved if all was fair and equal after the meeting, but I have now come to the opinion NHS Wiltshire CCG considers itself above the Law which it appears to break with worrying regularity ?

The top two people at NHS Wiltshire CCG appear to be abusing me by denying me my rights and services I am entitled to receive ?

Further to my FOIA request here that has been sent to NHS SCSCSU a week ago, I am aware you have previously experienced ' difficulties ' in receiving or dealing with my requests within the Legislatory time frame, and therefore trust you have no objection to my reminding you that NHS SCSCSU has not acknowledged receipt of my request of 05 11 2015 , or issued a reference number as should be the norm ?

Despite also sending this request AGAIN last August 2016,
by signed for recorded delivery to their acting Chief Officer,
and also writing again by recorded delivery regarding the matters mentioned within this request to the Chairman DR Peter Rowlands, neither has been acknowledged or responded to in any way.
This is all having a seriously negative effect on my health and wellbeing , I have been asking them for advocacy assistance that I am entitled to for several years now, but they appear to ignore my repeated requests.
No wonder suicide deaths of mental health patients in the community are up 50% in the past three years, if we all get treated in such a discriminatory and abusive way by those responsible for health provision in Wiltshire.
