Firstly, with regard to Q1, 4 and 5 the Trust produces tailored patient

Q3, please could you clarify this question? The Trust maintains patient

Thank you for your response to my initial request. All requests were based on information and procedures associated with day surgery patients. I have included my responses to your points as follows:

Your response: Firstly, with regard to Q1, 4 and 5 the Trust produces tailored patient information and guidance for different procedures as well as more general guidance that can be used for any operation. I have been advised that as the request stands these questions will cover 300+ documents.

My response:
Q1 – The information referred to is information given to the patient prior to an operation. I was assuming that there are tailored/specific notes, a sheet or two of A4, describing the specific operation and a sheet or two of general information, as you confirm, on the procedures to be followed during the time in hospital.
Q4 – The information referred to general information for the patient describing the outcome of the operation with, if provided, separate tailored/specific information on the operation and the likely recovery plus, possibly, some more general information including recommendations for future activities post discharge.
Q5 - This is the information supplied to or carried by the patient to their GP.

I can only imagine that the 300+ documents you refer to are the specific documents produced for the full range of possible day surgery procedures. If that is the case please forward the tailored/specific ones covering information offered prior to and after a selection of specific procedures, I have attached a list but feel free to chose any ten that are convenient for you.
In addition I would appreciate the general information sheets that will apply across the range of day surgery procedures.
My assumption is that the documentation provided for the GPs is standard. Please forward these forms.

Indicative list of suitable day surgery procedures:
Orchidopexy, Circumcision, Inguinal hernia repair, Excision of breast lump, Anal fissure dilatation or excision, Haemorrhoidectomy, Laparoscopic cholecystectomy, Varicose vein stripping or ligation, Transurethral resection of bladder, tumour, Excision of Dupuytren’s contracture, Carpal tunnel decompression, Excision of ganglion, Arthroscopy (all arthroscopic examinations of joints), Bunion operations, Removal of metalware, Extraction of cataract with/without implant, Correction of squint, Myringotomy, Tonsillectomy, Sub mucous resection, Reduction of nasal fracture, Operation for bat ears, Dilatation and curettage/hysteroscopy, Laparoscopy, Termination of pregnancy.

Q3 Having given the patient contact details for the hospital, which the patient has passed on to relatives, please can you provide procedures detemining how the relevant hospital personnel are instructed to maintain the lines of contact so that an enquiry either verbally via telephone or written by the family, about the patient’s location and/or state of health can be responded to.

While the Trust’s is only a partial response to the first point of my original request for information, there is no response or reference to the other points, namely (using the designation of the original request):

The Trust’s response (below) explains our procedure on these matters for day surgery patients. If it was unclear or is not the type of answer that you were expecting please could you clarify the question or state what you require as an answer and we are happy to provide this information.

‘All day surgery patients are accompanied by an escort, usually a friend or family member. When the day surgery patient is admitted, they have the option of providing the details of further friends, family members or an appropriate person for staff to liaise with if required.’

It is my understanding from previous clarification that this question related only to general information and advice. As noted in the Trust’s response general information is given verbally rather than in a document. All other information is condition specific.

I attach, below, a copy of my most recent response to the ICO.

To date the lack of response is a matter of concern. The only documents forwarded are information leaflets and cannot be described as procedures. The two links to procedures for the discharge of day surgery patients are from hospitals outside the ICHT and are not apparently followed by staff in the ICHT, so they are irrelevant.

Does the ICHT actually operate/function without written procedures? Do people make up what they are going to do without any written documentation? That, to me, is as unlikely as not having written job descriptions.

I am still requesting that you review my original FOI request and respond.

Thank you for your email, attached, and for the support you have given me.

Part of one of the early responses from the Imperial College Healthcare Trust (ICHT) referred to what I had asked for as requiring over 300 documents. My response was to pick out a number of possible specific documents to aid selection and for the ICHCT to be able to forward some representative documents. See Item A below.

To date the only documents forwarded have been general notes for patients which are presumably handed out once the patient's ailment has been diagnosed. This is not a procedure.

For any procedure to have any value, regular monitoring needs to be carried out to determine whether it is being followed. My questions were all aimed at finding our how the procedures were monitored, what the results of this monitoring are and any action that has been taken to ensure that procedures are complied with in the future.

The ICHT has simply ignored these points. In the ICHT hospital I was told that they have procedures for all the work carried out there. I would expect that there are procedures - or how does anyone know what they are supposed to do? Am I wrong?

I have discussed your request for information with our patient information
team and they have pointed out a number of points to consider.

Firstly, with regard to Q1, 4 and 5 the Trust produces tailored patient information and guidance for different procedures as well as more general guidance that can be used for any operation. I have been advised that as the request stands these questions will cover 300+ documents.

My Response of 19th May included the following:
I can only imagine that the 300+ documents you refer to are the specific documents produced for the full range of possible day surgery procedures. If that is the case please forward the tailored/specific ones covering information offered prior to and after a selection of specific procedures, I have attached a list but feel free to chose any ten that are convenient for you.
In addition I would appreciate the general information sheets that will apply across the range of day surgery procedures. My assumption is that the documentation provided for the GPs is
standard. Please forward these forms.

Indicative list of suitable day surgery procedures:
Orchidopexy, Circumcision, Inguinal hernia repair, Excision of breast lump, Anal fissure dilatation or excision, Haemorrhoidectomy, Laparoscopic cholecystectomy, Varicose vein stripping or ligation, Transurethral resection of bladder, tumour, Excision of Dupuytren’s contracture, Carpal tunnel decompression, Excision of ganglion, Arthroscopy (all arthroscopic examinations of joints), Bunion operations, Removal of metalware, Extraction of cataract with/without implant, Correction of squint, Myringotomy, Tonsillectomy, Sub mucous resection, Reduction of nasal fracture, Operation for bat ears, Dilatation and curettage/hysteroscopy, Laparoscopy, Termination of pregnancy.

Thank you for your email. I think that at some point in the process of
replying to your questions and trying to explain the Trust’s position
there has been a misunderstanding. While there are no written step by step
policies and procedures for giving patients information about their day
surgery, providing them information to give to their GP and communicating
with families these tasks are normal daily practice and such important
parts of the day to day running of the Trust’s services that all relevant
staff understand common practice as part of their role.

 

I think it would be of great benefit if we could talk through this on the
phone rather than attempting to resolve it by email. Can I ask if there is
an appropriate telephone number and time I could contact you? Or perhaps
you would like to ring me at the number below?
