Please note, as of 1 April 2021, NHS East Berkshire CCG, NHS North East Hampshire & Farnham CCG, and NHS Surrey Heath CCG merged to form 
NHS Frimley CCG.  The former East Berkshire CCG is made of up three Places: Bracknell Forest, Royal Borough Windsor and Maidenhead and 
Slough.
