A PAP involves an appointed medical specialist whose function it is to manage the PAP to a conclusion.
Do you have any similar processes ?
And I don't want to restrict your answer to any specific area of care.

This is the definition which is given by Western Trust:
""What is a Positive Action Plan?
A Positive Action Plan (PAP) is an individualised plan focusing on the systems/services around a patient.
The process of arriving at a written PAP involves reviewing past and present care and ascertaining what has and has not helped the patient to make positive changes in their life. It considers how various parts of the system/services connect and interact with each other and how they function in response to the patients assessed needs. It also considers the likelihood of positive change occurring in the future to the patients functioning as a consequence of changing these systems/services.
A PAP states what should and should not be done in the best interests of the patient.
A PAP is particularly helpful in complex cases, where there are multiple services and/or health care professionals involved. It states what the persons care needs are and who should meet these needs.
It ensures that care is well planned, consistent and based on the patients assessed needs. It ensures that all involved (patient and professionals) are clear about their roles and responsibilities, and clarifies how information should be communicated.
A Positive Action Plan is reviewed on a regular basis and should only be changed as part of a Multidisciplinary discussion."
