Policy on falsified notes and documents

Response to this request is long overdue. By law, under all circumstances, Aneurin Bevan University Health Board should have responded by now (details). You can complain by requesting an internal review.

Dear Aneurin Bevan University Health Board,
Can you tell me what disciplinary action is taken for falsified notes or documents? And is there a policy that you follow or is each case taken on its own merits

Yours faithfully,

Celia Jones
