Dear Mr Archer,

Re: Freedom of Information Request

I am writing to confirm receipt of your request for information from NHS North West London.

I am currently liaising with the appropriate Directorates to establish whether the PCT holds this information. The deadline for receiving a response under the Freedom of Information Act is 25 November 2011.

Under the Statutory Instrument 2004 No. 3244 The Freedom of Information and Data Protection (Appropriate Limit and Fees) Regulations 2004, NHS North West London is entitled to make a charge for copies of information held. I will contact you further if this is likely to be the case with your request.

Dear Mr Archer,

I apologise in the delay in getting a response to you.

I am still trying to ascertain whether we hold the information you have requested. I will continue to chase and get back to you as soon as I get a response.

As a point of note your request asks for some highly specific information. It is not likely that there is a special policy that meets all the criteria you have listed.

Many thanks,

Dear Mr Lall,

Many thanks for your message

I realise that the FOI request is highly specific. However, the circumstances are real and therefore, a policy will need to be formulated, if one doesn't already exist

In any event, I am mindful that this may take you a little more time than it would normally with another FOI request and as such, will wait for an extra period of time for you to complete your investigation

Yours sincerely,

Jeffery Archer

Dear Mr Archer,

I will pass on your email to the relevant directorate (if it exists within the organisation). I should point out, however, that if the information does not exist it is not an Freedom of Information 2000 obligation to create one.

I should also point out that as a Commissioner of Services rather than Provider of Services it is highly unlikely that we are the correct organisation to be requesting the information in the first place.

Many thanks

Dear Harrow Primary Care Trust (PCT),

Please pass this on to the person who conducts Freedom of Information reviews.

I am writing to request an internal review of Harrow Primary Care Trust (PCT)'s handling of my FOI request 'policy for Phonosurgery surgery for a female of transgender past'.

My FOI request is focussed on the policy for funding and referral and not on the medical suitability. Therefore, the commissioner of such services is the correct organisation to reply to this request. It is now long overdue

A full history of my FOI request and all correspondence is available on the Internet at this address:

Yours faithfully,

Jeffery Archer

staff numbers around so there may be some delay. We apologise for the
inconvenience and will endeavour to respond as soon as possible.

Dear Andrew Lall,

The attached document is for those who wish to change gender and are referred to a mental institution for examinations by mental staff. This patient is already legally and anatomically female and does not wish to change gender

Are you suggesting that this female patient will be referred to a mental institution for surgery for her voice?

Please clarify or attach the correct policy or state that no such policy exists

Yours sincerely,

Jeffery Archer
