Policies + Literature re: Gender Identity Services
To whom it may concern,
This is a request under the Freedom of Information Act (2000) in relation to the trust's service for children and young people with gender identity issues. I would be grateful if you could provide me with the following information:
1) Standard consent forms issued to patients and/or their guardians with respect to the treatment of Gender Identity Disorder (GID).
2) Internal policies or guidance relating to the diagnosis of GID either by the service itself or as expected of 3rd parties referring patients to the service.
3) Any questionnaires, surveys, or similar, issued to patients in the course of diagnosis, treatment, or evaluation of treatment for GID.
4) Any literature provided to patients and/or their guardians explaining GID, the diagnosis and treatment thereof, the availability of 3rd party support (e.g. charities, community groups), or similar.
5) Internal policies or guidance relating to the treatment of GID either by the service itself or as expected of 3rd parties as part of a shared care arrangement.
6) The names, participant bodies, and (where such exists) terms of reference, of any internal or external coordination or consultative groups relating to the treatment of GID in children and young people which the service hosts or participates in.
7) The names of any trans, LGBT, or equivalent charities / community groups with which the service regularly consults outside of any formal body covered by the previous point.
