|2| Policy on visiting times. Recent changes to visiting times and any notes on how this decision was made.

|3| Policy for Management of abusive towards staff. Zero tollerence polcy etc. The process of how this is handled and who ultimely is responsible.

I wonder if you could still please carry out an internal review.

The reason being, the document you have supplied deals with transfers between wards solely by means of a form. It seems literally incredible to me that this would be the extent of the process. I would have hoped there would be policies that address ensuring that the receiving ward has sufficient resources to support the patient, e.g. having enough beds; a process for handing over key needs and information to the new body of staff (as generally happens on shift change on wards), ensuring patient property is properly accounted for, and dozens of other important procedures and bits of information. Such handover surely can't be effected solely by means of that form. And whilst I understand the policy is under review, the existing policy must surely have been thought through with such vital practicalities in mind.

Please conduct an internal review to double check whether there is any other recorded information caught by the first element of my request:

Also if an alternatively worded request would likely result in more comprehensive or useful information in this area, please do let me know.

We write further to your reply email of the 24th August.

                                                                                             

Please allow us to address your further comments in turn below.

 

The reason being, the document you have supplied deals with transfers
between wards solely by means of a form –

In addition to the form, there is a verbal handover (nurse to nurse) prior
to the patient being transferred from one ward to another. The handover
form confirms all of the details that the receiving ward requires. This
will be more explicitly described in the refreshed document.

 

I would have hoped there would be policies that address ensuring that the
receiving ward has sufficient resources to support the patient, e.g.
having enough beds –

Bed occupancy is carefully monitored by the Patient Flow Team and this
allows patients to be moved from one ward to another when the bed is
available. Our intention is that each of our wards are staffed to ensure
the safety of all patients within its care;

 

a process for handing over key needs and information to the new body of
staff (as generally happens on shift change on wards), ensuring patient
property is properly accounted for, and dozens of other important
procedures and bits of information –

There is always a verbal handover between clinical teams from one shift to
another. This focusses on the main clinical condition and symptoms,
clinical management plans and other key information relating to how the
patients is, what they are able to do for themselves, what help they need
(eg. with personal cares, mobility, etc) and what other services are
involved with the patients care.

 

Such handover surely can't be effected solely by means of that form. And
whilst I understand the policy is under review, the existing policy must
surely have been thought through with such vital practicalities in mind –

The policy won’t describe every detail of handover but we will ensure that
the refreshed document more explicitly describes the process than the
current.

 

Please conduct an internal review to double check whether there is any
other recorded information caught by the first element of my request:

during the refresh
