Dear University College London Hospitals NHS Foundation Trust,

1) please can you send me a copy of the current subject access request acknowledgment AND response letter that you use
2) a copy of the last 5 dpias completed
3) a copy of any internal mandatory information governance training that you give to staff which was written in the last 2 years including presentation slides and videos and any other media
4) a copy of any instructions given to staff members to reduce data security breaches, for example double checking work which was written in the last 5 years
5) a list of any policies implemented in the last 2 years within the organisation to help reduce the environmental impact that the organisation has?

Thank you for your request for information relating to UCLH NHS Foundation Trust, received on Monday 16th December 2019. You should receive a response by Thursday 16th January 2020 at the latest, complying with the 20 working days as prescribed in the Act.

Occasionally, we may not be able to provide you with all the information you require by that date, but if this is the case we will inform you, stating the reason and the date by which we would hope to respond to your request.

Under the Act, there are 23 exemptions that can be applied to the information you request. This may mean that we do not have to supply some or all of the information, and in certain circumstances, do not have to tell you if we hold the information. Our Response:  We don’t use template responses as such but have attached
our subject access request form that we request people to complete and
return with appropriate evidence.

 

Your Request:  a copy of the last 5 dpias completed

 

Our Response:  Unfortunately we are still reviewing the level of
information to be released as they contain information that may be
sensitive to Trust security.  We’ll endeavour to send you a response in
the near future.
