1) please can you send me a copy of the current subject access request acknowledgment AND response letter that you use
2) a copy of the last 5 dpias completed
3) a copy of any internal mandatory information governance training that you give to staff which was written in the last 2 years including presentation slides and videos and any other media
4) a copy of any instructions given to staff members to reduce data security breaches, for example double checking work which was written in the last 5 years
5) a list of any policies implemented in the last 2 years within the organisation to help reduce the environmental impact that the organisation has?

Thank you for your Freedom of Information request which was received on
14.12.2019 by the Trust.

 

Following receipt of your request, searches were conducted by Sandwell and
West Birmingham Hospitals NHS Trust to locate information relevant to your
request and is detailed below.

 

1)      please can you send me a copy of the current subject access
request acknowledgment AND response letter that you use 

Response: Whilst we hold a response letter template, we do not hold an
acknowledgement letter template.  Please find a copy of our response
letter template attached.

 

2)      a copy of the last 5 dpias completed

Response:  Although there are a number of DPIAs currently underway, they
are still ongoing and as such have not been completed. 

 

3)      a copy of any internal mandatory information governance training
that you give to staff which was written in the last 2 years including
presentation slides and videos and any other media

Response: Information governance training provision is delivered using
nationally provided e-learning materials.

 

4)      a copy of any instructions given to staff members to reduce data
security breaches, for example double checking work which was written in
the last 5 years 

Response:  Please find attached a copy of the Confidentiality and Data
Protection Policy, Supplier Questionnaire and Cloud Supplier Questionnaire

 

      5) a list of any policies implemented in the last 2 years within the
organisation to help reduce the environmental impact that the organisation
has?

Response:  We realise that our healthcare services have a significant
impact on the environment and are committed to reducing these impacts,
whilst also working towards creating a more efficient and sustainable
system. We are embedding sustainability into the heart of our organisation
and working to use resources, like energy and water, more efficiently to
reduce wastage and save costs.

 

Our overarching aim is to deliver high quality care without exhausting
resources or causing environmental damage. We believe that investing in
infrastructure to improve energy and water efficiency will bring about
positive environmental impacts and cost savings. We also want to engage
our colleagues and have recently launched our ‘Green Impact’ staff
environmental behaviour change programme to get staff to take small
actions that will collectively make a big impact. Reducing energy and
water wastage, generating less waste, and travelling actively and
sustainably will benefit the environment and improve the wellbeing of our
staff and patients.

The circumstances under which they can reasonably expect to be provided
with heating and cooling temperatures used by the Trust, and

·         Get guidance on internal temperatures used by the Trust

·         Understand the process by which requests for heating and cooling
are made, assessed, approved and implemented
