you asked for the minutes of the National Panel Review, held on or around
10 September 2019, specifically in relation to the rating of

Please see attached the QA Tool for ECG Demand. This document is the tool
we use for recording our notes on from the panel meeting

 

We attach copies of this information.

 

You will see that the names of some CQC employees have been removed to
protect their confidentiality.

 

We also attach a formal note, giving a more detailed explanation for
withholding this information. This explains why the information is exempt
from disclosure under the Freedom of Information Act 2000 (FOIA) and your

I cannot locate the attachment containing the QA Tool which you say contains the minutes.

I write in relation to your correspondence of 13 November 2019 whereby you requested an internal review of the response to CQC IAT 1920 0538.

My manager and I have looked into this matter and I can confirm that due to human error, the information which I believed to be attached to the previous email was not. This was a sincere oversight on my part for which I take full responsibility.

I uphold your complaint about our handling of your request.

 

I apologise that the QA tool referred to in our response of the 8 October
was not attached. This was a human error on behalf of the Officer who
handled your request.
