PIPs department system down on Wednesday 11 May 2016

Re: PIPs enquiry line (0345 850 3322)

On Wednesday 11 May 2016 at around 5:30pm, an adviser on the telephone informed me that the system to access case files was 'down'.

I am hereby requesting the following under the FOI act:
1. Please provide any/all correspondence circulated to staff regarding the 'downtime', including all correspondence in regards to the initial report of an error (if applicable)
2. Please detail any reasons of why the system was 'down'
3. Please disclose staff guidance and DWP procedures for such events when a certain computer system is 'down'/not working

I would be grateful if you could please respond to my request via PDF letter.

I SHOULD of received my request by: 22 June 2016 (this excludes non-working days and holidays)
I chased on: 24 June 2016 (2 days overdue)
I am following up again on: 28 June 2016 (3 days overdue)

Please pass this on to the person who conducts Freedom of Information reviews.

I am writing to request an internal review of Department for Work and Pensions's handling of my FOI request 'PIPs department system down on Wednesday 11 May 2016'.

I originally sent my request in on 24 May 2016 meaning I should of received a response by 22 June 2016. I am still awaiting a response. This is now my THIRD reminder to remit a response ASAP as this is now heavily overdue.

I would like an explanation from the person who conducts the internal review to as why this particular request has been mishandled and delayed for so long. Please do not simply just apologise, please explain why the delay has happened.
